This program is for people who have experienced a cardiac event, cardiac surgery, a heart disease related episode or for those people who have risk factors and wish to prevent heart disease.  We follow the evidence based Cardiac Rehabilitation guidelines as set by the Heart Foundation. The evidence has proven that people who attend Cardiac Rehabilitation after a cardiac event will decrease the risk of having a life threatening event in the future by 30 per cent.

Program Details

This is a six week course of two hours each week.

Clients will first be asked to undertake an assessment to establish their personal health goals and determine if they wish to participate.

Clients will participate in an individualised and supervised exercise session for the first hour of each week; the program is run by a Registered Nurse and a Physiotherapist. This is followed by a one hour education session each week in a relaxed atmosphere facilitated by a health professional.

Education topics include:

  • Medication management
  • Symptom management
  • Healthy eating
  • Physical activity
  • Stress management and relaxation.

Eligibility

A hospital, Cardiologist, Cardio-Thoracic Surgeon or a GP referral is required to access this program.

Availability / Times

Initial assessments take place at Kyneton Toyota Sports and Aquatic Centre on Tuesdays.

The Cardiac Rehabilitation Program is on Tuesdays from 11:00am-12:00pm (Exercise – Kyneton Sports and Aquatic) and 12:00pm-1:00pm (Education – St. Luke’s Kyneton)

How to make a referral

A GP or hospital referral will contain a request for service, past and current medical history and medication, any contraindications for treatment and alerts, and any relevant radiology and pathology reports.

It is preferred that agency/service referrals are received via www.connectingcare.com
