Dietetics

Dietitians assess people’s nutritional status and provide food and nutrition information to improve health and wellbeing. Dietitians provide information about modified diets to manage conditions such as malnutrition, dysphagia, diabetes, heart disease, obesity, cancer, food allergies and intolerances. Dietitians assist people by providing dietary and nutritional advice to assist nutritional wellbeing.

We accept referrals for conditions such as:

  • DietiticsUndernutrition
  • Diabetes
  • Heart Disease
  • Coeliac Disease, FODMAPs issues and other bowel conditions
  • Weight Control.

Eligibility

  • Younger people with a disability (HACC Program for Younger People)
  • Non-complex Paediatric clients
  • Medicare, DVA and Chronic Disease Management Plan referrals

Commonwealth Home Support (CHSP) Eligibility

  • Frail, older people aged 65 years and over (or 50 years and over from Aboriginal and Torres Straight Islander People) who need assistance with daily living to remain living independently at home and in the community
  • Frail, older CHSP clients aged 65 years and over (or 50 years and over for Aboriginal and Torres Straight Islander people) who receive planned respite service, will allow regular carers to take a break from their usual caring duties
  • Frail, older people or prematurely aged people 50 years and over (or 45 years and over for Aboriginal and Torres Straight Islander people) who are on a low income and who are homeless or at risk of homelessness as a result of experiencing housing stress or not having secure accommodation

Program Fees

Dietetics is funded by HACC Program for Younger People, Commonwealth Home Support Program, Department of Veterans Affairs, Medicare and Private Health.

Community Dietetics operates between 9am and 5pm (Wednesday to Friday)

How to make a referral

Contact Intake 03 5421 1666.

A GP, Hospital or Health Practitioner referral will contain a request for service, past and current medical history and medication, any contraindications for treatment and alerts, and any relevant radiology and pathology reports. To help to prioritise referrals, it is important to include as much information as possible.

It is preferred that agency/service referrals are received via www.connectingcare.com

If you are referring yourself, the Intake Worker will ask you some questions over the phone when you contact us.
